Common partition-table mistakes

App partition too small: with OTA enabled the firmware must fit in half the flash.
Offsets must be 4 KB aligned: hand-editing CSVs is where this usually breaks.
NVS too small: 20 KB is a safe start; go higher if you use Bluetooth provisioning.
